TO OBTAIN CURRENT INFORMATION ON ALTERING YOUR UNIT CONTACT A BOARD MEMBER.  

This form is required for ALL proposed improvements.  Painting, decorating, blinds, new carpeting (any level unit), new cabinets, new counter tops, new tubs/showers that utilize the old drain and water supplies (like-for-like installation), new lights that utilize the same electrical wiring as the old fixture, new appliances and minor changes require only submittal of the form so board and management can stay informed, but do not require board approval.   IMPROVEMENTS NOTED BELOW DO REQUIRE BOARD APPROVAL.  On the form below describe your project and then submit the form. You will be notified if your project does not require board approval, and if board approval is required you will be notified if/when it is approved (allow up to 30-45 days if approval is required). 

Any alterations that will require water to be turned off requires notification of neighbors 24-48 hours prior to the work beginning and notification of AMS NW, Samantha Linam, samanthal@ams-nw.com 48 hours before construction. Water shutoff locations are posted in the building lobbies and can be found on this website at emergencies.

As a courtesy to neighbors, they should be notified 24-48 hours before any activities that will involve significant noise, ​​​​​dust, vestibule traffic or other inconvenience.

LIfetime Exteriors are to be contacted for any project involving exterior siding/trim, exterior doors and windows.  They are the holders of the warranty for these improvements and they review any construction to verify the work will not damage these components and possibly void the warranty. If your project involves ANY of the following the form must be submitted AND approved by the board prior to the start of construction (allow up to 30 to  45 days for review/decision):

-Hard flooring (wood, tile, stone, etc) on a second floor. Special conditions must be met.       Soundproof underflooring and maximum coverage of 75% by hard surfacing. Submit a drawing showing where hard flooring is to be installed.

-Altering/adding an external heat pump. Requires approval/coordination with Lifetime Exteriors. Submit letter of coordination with Lifetime Exteriors.

-Any changes that impact the exterior walls, roof, windows or exterior doors of the unit (attaching a golf screen, awning, canopy, shade, dish, satellite, security camera, solar tube, additional wiring on exterior for cable, electric or other utilities, etc that will require drilling and/or attaching to the any exterior surface. Requires approval/coordination with Lifetime Exteriors.  Submit letter of coordination with Lifetime Exteriors and plan of proposed changes/additions.

-Structural changes (moving, removing walls) and major repairs of leaks require certification by a structural engineer or licensed architect. Submit plans and engineer/architect certification.

-Alterations that will require plumbing or electrical changes (moving outlets and switches, running new wiring, connecting/changes in electrical panel, changing size or location of water supply lines or drains). Submit plans for changes and certification from licensed contractor.

-Any changes that require a city building or structural permit.​​​​ Submit copy of approved permit(s).​​

Complete the form and click on the SUBMIT button.  Be certain to completely describe the scope of work for the project.  The form will be forwarded to all board members and Association management.  You will receive an acknowledgement that the application was received for review.  If approval is required you will be notified if the project was approved.
